Do you know someone who outplayed, outran, outscored, out-performed every competitor? Anyone who ran the extra mile/went the whole nine yards/covered every blade of grass/left it all on the pitch...?

The Hackney Sports Awards 2011 are taking place on Wednesday 2 November 2011 at the Hackney Empire. This is your chance to nominate someone you know who has been outstanding in their discipline, or has been part of an amazing team.

There are ten categories in all:

  • The Mayor's Award
  • Inclusion Award
  • Young Volunteer of the Year
  • Coach or Instructor of the Year
  • Voluntary Services to Sport
  • Sports Club of the Year
  • Team of the Year (under 11 and under 18)
  • Sports Personality of the Year (under 13 and under 21)

How do you nominate someone? Easy, just download the form and return it to: Leisure & Physical Activity team, Hackney Service Centre, 1 Hillman Street, London E8 1DY
